About the project
CHANCE aims to improve health literacy among migrant youth and their communities by providing accessible, culturally sensitive information on healthcare rights, sexual and reproductive health, and mental well-being. The project will reach at least 200 young migrants and community members, empower 24 peer mentors from migrant communities as trained health literacy ambassadors, and strengthen the competences of 12 youth workers in delivering inclusive, trauma-aware, and culturally responsive health education.
Key results include a Health Literacy Toolkit with practical methods and multilingual materials, a Peer Mentor Training Pack for youth organisations and NGOs, awareness-raising materials co-created with young migrants, and a project website providing open access to all project resources.
The project aligns fully with Erasmus+ priorities on inclusion, diversity, and innovation, supports the EU Action Plan on Integration and Inclusion and the EU Youth Strategy, and contributes to the UN Sustainable Development Goals on health, education, and reduced inequalities.

Objectives
The following objectives outline the main goals and direction of the CHANCE project.
Imprοve health literacy of at least 200 migrant youth and community members by providing accessible, culturally sensitive information on healthcare rights, sexual/reproductive health, and mental well-being
Empοwer at least 24 peer mentοrs from migrant cοmmunities by training and certifying them as health literacy ambassadοrs whο will actively suppοrt community οutreach until the end οf the project
Equip at least 12 yοuth workers by strengthening their cοmpetences in delivering inclusive, trauma-aware, and culturally respοnsive methοds fοr delivering health education

Direct Target Groups
Young migrants (18–30):
Main target group engaged through workshops, focus groups and campaigns to improve health literacy, awareness of rights, sexual and reproductive health, and mental well-being.
Peer mentors from migrant communities:
24 young people trained as “Community Health Champions” to provide peer support and raise awareness.
Youth workers, educators and NGO staff:
12 professionals trained in trauma-aware, inclusive and culturally responsive health education methods.
